Building is 1/2 mile north on the left in the Dowd Insurance building. Federally certified DOT medical examiner #2578878056. We have certified thousands of drivers locally and from across the nation in a timely manner, in a low stress environment. Appointment system means you are in and out generally in less than 30 minutes. Exams are $120, discounts available for military and first responders. Free laminated medical card. Can never get a hold of either of the locations to make an appointment.. I been calling everyday to both locations at different times of the day never getting through. I have left numerous messages to never getting a call back. I will never go here where they show no interest in answering the phone or responding to messages. EDIT That was this week 2 weeks ago you was on vacation. For the last 3 months I have been calling everyday to make an appointment but there has never been a answer I have also left 5 or so messages saying I wanted to set up a DOT Physical but no call back.. I keep getting people suggesting your place to get my DOT Physical done and your website states can only do appointments so I call and I get no response. I have tried for over 3 months to get one. You can not use this "Summer Break" as a reason for the past 3 months of not returning phone calls. every time I left a message it was when the message was "We are either with a patient or out of the office" I was that's fine and okay with me. So I left my Name, purpose and my phone number in the message..
Great place to go for a DOT physical you are in and out and the Dr is really friendly with no hassles. Strongly recomend I have been going to him for years now.

Dr Mark Czerniak is a very professional, kind, and no-hassle kind of guy. Very close to the Mass pike, easily accessible by car, and bobtails could probably park at the station next door. I would highly recommend to anyone needing a DOT physical.
Highly recommend Dr Czerniak to any drivers who need a quick, inexpensive, hassle free DOT Physical. Dr. Czerniak was fast, friendly, knowledgeable, and considerate. In my case he got me in for a Physical within 24hrs of my phone call. If you are in the Springfield, Ludlow, Palmer, Wilbraham, or Belchertown area I would definitely recommend going here. The office is small, private, and clean and super easy to get to from anywhere. I came from Palmer and it was a 12 minute drive on back roads or you can get off Ludlow Pike exit, turn left and you’re there.
